You can buy over the counter online or a number of hormone clinics sell them
If you go to a clinic there is usually a check up. Some will have it available to you to buy and others will only show you how to use and tell you where you can buy it. It's definitely a grey area right now.

In my experience elbows can be an overuse thing and then I get it when I do exaggerated ROM like on curls and stuff
