Second offense should be an ejection just like every single personal foul penalty.

But targeting should be viewed as a malice and deliberate move to use the head as a means of making a tackle. Incidental hits and quick hit/collision plays should not be viewed as targeting. Also if an offensive players decides to drop their helmet to waist level and the defender matches that level and there is helmet contact, that should either be targeting on offense or no targeting at all.
