1. My wife would feed the baby every 4 hours (with a 45 min. adjustment either way) during the day. She would do this even if she had to wake the baby up from a nap to feed her/him. This seemed to get the baby on a normal wake/sleep schedule.
2. We always swaddled the baby in a thin blanket. The swaddle was pretty snug. Don't use extra blankets. They just get in the way and the baby could get too hot.
3. We used a white noise machine.
4. Our kids never slept in our bed. In fact, they never even slept in our room. They were in a crib in their own room. I bought a really comfortable recliner and put it in the baby's room for when my wife would need to nurse the baby at night.
5. Except for the first night with my first child, we never used a baby monitor at night. It wrecked my wife's and my sleep to hear every noise from the baby monitor. We didn't use it after that first night and never looked back.
6. We did the same bedtime routine (feeding, bath, lotion, clean diaper, pajamas) nearly every night. Sometimes that last feeding before bedtime would be formula since it creates a bit larger curd in the stomach and made the baby feel more satiated.
7. We put the swaddled baby in the crib, turned off the light, and closed the door.
I certainly don't have all the answers, and we may have just "hit the lottery" with babies who slept well, but these are the things that worked for us. Each of our children slept through the night (10 pm-6 am) before they were six weeks old.
