In this case it's probably because a primary sebaceous carcinoma of the
Eyelid is an insanely rare diagnosis. Sometimes very rare clinical syndromes are difficult to diagnose.

Was this doctor a Dermo or primary care? Dermos would be expert at spotting this stuff, as would opthalmologists
