They now request it annually right around the expiration. Even though I live in a free standing unit, I am in an HOA that is required to carry insurance on our structures...long story.

Anyway, the last two years they processed my submission incredibly slowly and then sent a letter telling me they were going to increase my monthly payment for the new insurance they picked up. An irate phone call immediately followed by a pointed email put the issue to rest.

What I learned...
Promptly respond to the very first notification.
Keep track of your notes and your submissions.
They will take off the insurance if you can prove there was never a lapse in coverage.
