Here's one a team actually used against my team in the 9th grade that worked:
Team runs a sweep to their team's side of the field. Team quickly huddles in the middle of the field. Ref places ball on the hash. We line up in front of the ball on D. Team in the huddle starts yelling for a player to come to the huddle from the sideline. As he's running onto the field, he reaches down and scoops up the ball and flips it to the QB who is standing behind the huddle. Even though we had a couple of guys over on the other side of the field...there wasn't enough to stop them from going 60 yards for the TD. It was a bush-league play and we still wiped them out 45-7.
