TBH, doctors and offices don't mind it at all when you do, either. They typically have a non-insurance rate and they're happy to not have to deal with the mess of filing. It's also nice for you because - to counter the point of a few in this thread - the offices or providers can give you a clear cost for the procedure/visit, whereas with insurance they can't because they have no idea what insurance will adjust.
I am no fan of Obamacare; but one thing I do like that was never fully implemented was cost transparency.
