not with insurance. There's nothing requiring you to submit insurance. They can choose not to work with you if you don't have insurance, but you're not required to give out insurance info. But honestly it's none of their business if you have insurance or not. If they ask you can choose to not disclose if you are really worried about lying. If they have a policy of not allowing cash payment if you have insurance and you really don't want to violate that then you just have to find another provider.
