I asked a counselor in my stake presidency...
About fat people getting temple recommends and the word of wisdom question. He said "Don't project your personal revelation about your own health onto other people."
I learned that my own approach to the "principled" parts of the word of wisdom is between me and the Lord, as is that of other people. The bigger issue was me judging other people's perceived unrighteousness.
You'll notice a couple of the Apostles got really fit after becoming Apostles (Renlund/Saurez). That was between them and the Lord.
