The QB can hear it, but most of the time the WR don't come within earshot of the QB so they have to signal anyway.
That's why there was a push from some instead of helmet comms for a wearable like a wristband that all the players wear that they all can look at individually. Only with something like that could you actually get rid of sideline signalling completely.

Except the majority of teams still go no huddle and have to signal in from the sideline just like they did before.

Or they lost their QB, their (to be) NFL RB, their NFL WR, and two NFL OL and their only spark last year (Vaki at RB/S)
