Definitely poor gap coverage & overrunning the QB/RB in effort to make plays, so
your option 3 is the predominant reason; however, Gundy's scheming and extra preparation time definitely gave them and advantage and set the stage for early success and getting our guys on their heels...ergo the two are intertwined.
