You can come shadow me for a day and I'll show you why it works
My field is a pretty niche engineering field. There are a couple things that allow me to do lots of other things during the work day:

1) Meetings. I will summarize by saying: I can, and frequently do, call in from anywhere. Most meetings involve me listening and making some comments or giving status updates. I can easily make a sandwich, drive to pick up my kids, load some laundry, ... while listening to a meeting and chiming in when necessary.

2) Compiling code. Building one of my designs takes about 1 hour on average. It's what I'm doing right now. I have a design building and it's not going to be ready to test and debug for an hour. My employer can't "trim the fat" to fix this problem. You could fire me but then nothing would get done. You can't hire more people and speed up the build times. You can't hire different people and speed up the built times. It is what it is. So I can send a message on CB, I can go mow the lawn, I can make a shopping list, I can do some research for our trip to Ireland, ...

It's easy to understand that there isn't an equivalent thing in the dental world. You have to be in-person. Your office is busy all the time. You're visiting patients, teeth are getting cleaned, x-rays read, ... It's busy and there's really not a lot of downtime. I get that. In my world, there is kind of a lot of downtime. I've worked a bunch of places and it has existed at each one. It's a nice perk. My employers have never given me anything but glowing reviews of my work. They're happy with me and my process. I am sought after as a consultant and contractor in my field on top of my full time work. I'm not just slacking off. It's just the nature of the work.

But yes, many days I have enough downtime that I could go watch an entire movie, on the clock, while things are building without any noticeable disruption to my productivity.
