Just took a group of students to those places. Some thoughts:
1. Yes, get a tour guide for this, Rainbow Mountain, etc. They're cheap and well worth it. They usually work very hard for you, pointing out interesting things along the way, stopping at sites, driving fast to get you there, etc. We've used Adios Travels and Lamanita Travels and really appreciated them.

2. For Sacsayhuaman, getting the Cusco pass is a good idea, and you can catch an uber there on your own. It'll look like just a bunch of rocks, though, but if you get a guide, they'll explain the interesting things about the Incan designs in the rocks. Highly recommend a guide. If you don't get one ahead of time, there are often guides at the front gate waiting for walk-up customers. Level of English ability may vary with those however.

3. My favorite place in the Sacred Valley you didn't mention is the weaving markets at Chinchero. Definitely make sure that's on the itinerary ... it's very cool to hear how the Incan ladies do all the textiles. They can tell you the stories in English. Make sure to buy from them, the tour is free but their money is in selling to you.

4. San Pedro is the largest market in Cusco for getting things cheaper, or just seeing variety.

5. At the salt mines, get the smoked salt. I brought home like 5 lbs of it. So yummy!

6. In Cusco and Aguas Calientes, eat alpaca ... I think it's the best meat. So yummy! Visit the sun temple.

7. Pachapapa is a really good restaurant. Chakruna alpaca burgers is also really good!

8. Chocolate is good there. Buy several varieties. My favorite is Iberica, which is from Arequipa, but see if you can find it there. They are the fondys and milkys in the grocery store.

9. For altitude sickness, drink coca tea every day. Hydrate lots.

10. Bugs can be bad (they were bad one time, not the other time, so bring repellant)

11. Rainbow mtn at the end is a good idea for the elevation. Take it easy your first day in cusco because of the elevation. Take sodium naproxen.

12. Watching the game? I have no idea.

13. Read last days of the Incas before you go and you'll understand so much of what you're seeing. Have fun!

14. Tipping is important. About 10% for your guide, 5% for your driver, 5-10% at a nice restaurant, 20 soles for your hotel service people, etc.
