It's important and necessary in my opinion if called for
by the manufacturer. Higher compression engines can cause the gas to self-ignite. Higher octane fuel increases the amount of compression that the fuel can take before self-igniting. It's really the only difference between high and low octane gas, but it matters if you have a car like a lexus that has a high compression engine.
