I was a tech in mental health and heard rumors of restrained or suicide watch
patients who were not monitored properly and died.
I think it was pretty typical to CYA and bury stuff like that for people especially who didn’t have family. I guess that’s a negligent homicide not a murder exactly

Had a client years ago that was well known in town. He was found dead in a Motel and it was classified as a OD.
