I believe the right answer is it depends
Generally speaking being an organ donor is a program you opt into voluntarily, not opting in isn't a decision on it one way or the other. Opting into being an organ donor, like when you get your license, is different than having a medical directive regarding your organs. The medical directive is generally definitive on what happens. Unless you have definitively directed something to happen to your organs, one way or another, your family will still be the ones deciding.
