1-3x in my life. 2-all the time on hills in my manual. 3-never. 4-5 all the time
But usually by a predetermined amount. 5-7k instead of 3-5k for oil changes. 20% more for scheduled maintenance. 6 - not a big deal and there are reasons to do it in purpose.
