I think it is a 3 day notice to quit or pay rent— followed by a court hearing in 15 days if notice is not complied with:

I think the Landlord has to file suit for unlawful detainer/restitution after the expiration of the 3 day notice.

For failure to pay rent, yes. But the LL still must get an eviction order.
