If it were me, I'd go in.
I'm an addiction psychiatrist, not a surgeon. Sudden change and pain severity make me think that it's something that should be examined sooner rather than later.

I would go to ER, but some are against that because of the copay. But urgent cares are pretty useless for serious stuff
