I agree with you that LDS culture likes to distill the WOW down to just 3 banned substances, ignoring the other aspects of it.
But, I don’t think it’s fair to conflate the LDS cultural simplification with the actual worthiness questions as posed. It asks if one observes the WOW, not specifically any particular portion of it directly, so the whole of it is used for worthiness inquiries, which would include the “do” tenets, not just the 3 cherry-picked “don’t do” tenets.
