it doesnt represent an "official form". It's some reddit nerd that creates this graphic each week and he entered in the wrong info. Not the first and wont be the last time he's fat fingered his graphic. It's a lot of info and its put together by a random dude.

It's kind of like if someone on CB created a graphic and you found something wrong with it...its not a conspiracy or even surprising, it's just an amateur putting together info and made a mistake
