I'm not a Disney fanatic, but I've always enjoyed going. Cruises, on the other hand, don't hold much interest, but it would be silly to call the people who enjoy them mentally ill.
I think the "mental illness" you're seeing comes down to a few factors.
1. A lot of people love dressing up for Disney in the same way some people have their Halloween parties. They are overly visible at the parks because of their outfits. That doesn't mean they dress with Mickey Mouse ears once they step outside the park, any more than Green Bay Packers fans go to work with a big block of cheese on their heads.
2. You're looking at thousands of people at a time. In any group of thousands of people there are going to be some people with challenges.
3. People who hate Disney for political reasons or for the fanaticism it inspires in others amplify the outliers by memeifying the goofies of the Disney fans.
