Call other driver's insurance and make a claim directly with them.
If you need to go to the doctor, go. Once you finish treatment (if any) or determine the extent of your injuries and what treatment you WILL need send all of that to the insurance carrier with proof of what the bike would cost to replace. Hope they pay for it all.
