It was a combination of a horrible back pass, a central defender who was in the right position but somehow couldn't put any pressure on the attacker, and Hernaez not making a huge play. It was a bad back pass, but that was still 45 yards from goal with 1 central defender still behind the ball and gotta expect central defender or GK to help clean that up.
