I've rented in Italy and agree with the pros listed by Korihor. For big cities

like Rome and Florence, just find a parking garage for the time you are there. At one time, there used to be restrictions on taking the car into certain parts of southern Italy or insurance restrictions for those areas. I'm not sure if that is still the case.
