NYC and Japan All-American
I hope you aren’t speaking of Japan healthcare from personal observation
I lived there 3 1/2 years. Lived all over the country. Every hospital I visited was scary. Run down, old… Foreigners went to great lengths to not receive healthcare in Japan, including now elder L. Todd Budge of the Presiding Bishopric and family (we were in a bishopric together my last time living in Japan).

After many bad experiences missionaries with serious health issues were flown home for treatment.

A neighbor is a PA. His daughter and my daughter were best friends. He, wife and daughter all served missions in Japan. His daughter had a minor medical issue. After 6 weeks in the hospital with little to no progress he was buying a ticket to Japan to fly her home when she finally started to improve.

A long time friend, native Japanese, spent 6 months on strict bed rest for a simple arm fracture on doctor’s orders.
