People are increasingly willing to pay for premium seats either with cash or points. Then they go through their list of non-revs and elite travelers. There's some discretion there. For example: if you have one business seat and the next person on the upgrade list is a couple traveling on the same reservation, sometimes they get skipped without offering to upgrade only one of them.

But nobody is upgrading the non-status guy who chatted up the gate agent.
