I'm a physician transitioning to this model now and these are my thoughts
1. Physicians don't have the time for it. Honestly we have 15-30 minutes to hear your complaints, read through your last hospitalization, check your labs, check your meds, check recent specialists notes, check and update your meds, order more tests, try to explain things in a dumbed down fashion and then document in a 5 page report everything we just did. That in 15 minutes. Repeat 25x a day. Our healthcare model in no shape or form is set up to properly care for true prevention (or reimburse for it).. At least outside of a handful of really, really good primary docs that are already stretched super thin who do their best with this.

2. These are conversations and interventions that take time. You need 1-2 hours to really break down expectations, their experience, what they're doing, what are their barriers, etc. You need close follow up. You need direct physician or "health coach" access. Again, just not set up with our healthcare model.

3. Physicians aren't experts in this. I'm a cardiologist that has had one lecture on nutrition in my 11 years of training/education. I don't recall one lecture on exercise. Everything I've learned is through my own background or on my own. It's really crazy when you think about it.

4. You need physicians to be experts in what they are. Regardless of what people do to prevent things, you'll need experts to bail them out.. even if that's just the effects of aging in completely healthy people. We are a very very limited workforce and you'll need me at 2 am in the middle of the night for your heart attack. So there are limited resources (again in our current model) so these gazillion dollar hospital corporations need to come up with better programs.

5. These fields (exercise/nutritional science) don't always have great data and with so many bro-experts, podcasters, that it's hard to tease out the nonsense.

Ideally.. and we will see how this goes, this is a model for private health or concierge type models where you have physicians on retainer that can spend 1-2 hours with you as needed and who can deep dive into all of your habits. If you have someone that you trust and that knows there stuff on medicine and lifestyle, then ideally you can have someone really help with your health... otherwise you're on your own.
