I think demand for change would come sooner if we had some laws prohibiting employers from covering it and instead just forcing everyone to the open market. Ii originated as just a perk for employers to woo employees, there's absolutely no reason it needs to be part of employment, but insurance companies sure do love how it means they can hide things and charge more.
