I think in unbroken they talk about how the overwhelming majority of
casualties were from plane malfunctions, pilot, error, etc.
They were rushing out crazy advanced technology and young men to handle it at alarming rates.

Not casualty rate. 8th air force bomber crew people had a 65% casualty rate. Second only to uboat crews.
