1) incentivize patients to see small private practices (such as myself, and this is where my bias comes in). Decentivize going to the IHC’s of the world where costs are higher.
2) Give a discount rate to those with health markers in a certain range. Such as A1C, smoking status, cholesterol, weight. That way those are who are unhealthy due to choice and a future drain on the system are paying a higher premium just like someone with a speeding ticket history is paying more for car insurance.
3) Simplify the billing process. If a claim is denied and then appealed and reversed then there should be an increased fee to discourage blanket denials. Cut out the “in network” crap. That unnecessarily hinders patient selection and benefits large corporations who strike deals with insurance companies.”
One thing I would add is that instead of government subsidizing young healthy families with no pre existing conditions. Subsidize those with pre-existing condition, and if you made the decision to drink, smoke etc well the consequence of your poor choices is a very high premium.
