Part of it is the govt/insurance playing stupid games just to minimize paying out anything requiring complex coding and gamesmanship... part of it is the pure greed of for-profit and not-for-profit hospitals/agencies that are layered in administration or c-suiters looking to get every penny they can get. What else are they going to do all day.

These guys obviously had a meteoric rise so probably were extra sleazy but ultimately what probably led to this is just the huge public numbers of their payout and resentment for their lifestyle.

This is probably American Gangster and a mink coat thing- though bet Jason gets out of this clean and with money intact.
