In our system, IHC or the likes has a nutritionist, exercise physiologist and a
counselor staffed in the clinic, so on an annual physical the patient would spend 30-60 minutes with each with each compiling their recommendations prior to meeting with their physician. You would then have an hour for 1:1 physician time. You would then have monthly check-ins via tele visits with each one.

Obviously no one (govt, insurance, patient) wants to pay for that.. and no one (IHC/hospital, clinic, etc) want to spend the money they do get on that.. so it's a mute point.

So definitely a system not physician problem/issue, though like everything the physician as the face of medicine gets blamed.
