You absolutely know when the screw has purchase.
I once saw a patient with bone so hard the surgeon broke an 8.5mm screw, then broke a 9.5mm in a different location. That was an interesting case because the patient was transgender and it was likely the hormone therapy that caused the abnormal bone density.

Conversely, I've also seen patients with bone so porous screws are still moving months post-op.
