Question about fake field goals
If the placeholder receives the ball and his knee is down, is there a special rule that allows him to not be down?

Yes it's an exception directly written into the rulebook

The rule is a kicker must be in position to kick the ball.

Could Ferrin have punted if a defender appeared to have impeded a 1st down?
